Comorbidities of someone with a gluteal tendinopathy
Typically female age > 40
Metabolic conditions
History of statins, anabolic steroids potentially, and glucocorticoids or other things that may weaken the tendons- ex fluoroquinalone
Symptoms of someone with a gluteal tendinopathy
Lateral hip pain
Pain with higher demand single leg activities- walking up hill, stair climbing, and standing on 1 leg
Pain with compressive loading of the hip- IT band will compress it and can occur with sitting with the legs crossed or sleeping on the painful side or on c/l side
Questions to rule up a gluteal tendinopathy
Changes in training load
PAin with stairs/hills or balancing on 1 leg
PAin with sleeping on your side
Gluteal tendinopathy examination
+palpation tenderness- +PPV- 78%
+resisted hip abduction- +PPV- 96%
+SLS- pain withing 30 seconds can use finger for support- +PPV- 99%
+FADER Test- Flexion at 90, Adduction and ER - + pain proviocation
+FADER- R test- Same position but add resisted IR at end.
- FADER test- .3 sensitivity, .87 specificity
- FADER-R test - .44 sensitivity, .933 specificity
Rule down hip OA
Evidence informed interventions for a gluteal tendinopathy
Education on reducing loading - reduce hill walking or running. Adjust sleep position.
Exercise- Think of progressions of tendon loading- isometric-istotonic and energy storage (plyo phase)
