A
  1. Not routienly drawn
  2. Therapy longer than 7 days
  3. BMI > 40
  4. SrCr over 40 umol/L. SrCr increases 50% over baseline
  5. Altered Volume of distribution (Cystic fibrosis, burns)
  6. Severe infections that require a good level

A

Dose:
- 2g IV loading dose right away. After 12 hours start 1g IV q12h (infused over 1 hour)
- Draw trough level before 3rd or 4th dose
