Complications of GORD
Oesophageal inflammation/oesophagitis
Ulceration
Haemorrhage
Stricture formation
Anaemia
Aspiration pneumonia
Barrett’s oesophagus
What is non-erosive GORD?
Normal endoscopy but symptoms suggestive of GORD
What is erosive GORD?
Mucosal lesions/ oesophagitis apparent on endoscopy
1st line for endoscopy confirmed GORD?
PPI for 4-8 weeks
2nd line for endoscopy confirmed GORD?
H2 receptor antagonist
Treatment for severe oesophagitis?
8 weeks PPI
Options if no response to 1st and 2nd line treatment for endoscopy confirmed GORD?
Add another month of PPI treatment, double the original PPI dose for another month, or add h2 receptor antagonist for nocturnal symptoms/ when required
When is long-term maintenance recommend in GORD?
If patient diagnoses with endoscopy confirmed severe oesophagitis
1st line therapy for GORD in pregnancy?
Lifestyle advice
2nd line therapy for GORD in pregnancy?
Antacid and alginate
3rd line therapy for GORD in pregnancy?
Omeprazole
