Why is this called a diagnosis of exclusion?

A

Symptoms can be similar to other conditions such as orbital cellulitis, dacryoadenitis or an orbital tumor. Want to rule those out by CT scan

Rule out inflammatory disease based on history

What is it?

A

Acute inflammation of soft tissues in the orbit
Could be of the fat, CT, muscle (myositis), lacrimal gland (dacryoadenitis).
Could also extend into the globe- tenons, sclera, or uvea inflammation.

Eyelid swelling, redness, pain
Red eye 
Bulging, displaced eye 
Double vision 
Pain on eye movement 
  • similar to orbital cellulitis

Eyelid edema, tender and painful 
Conj chemosis and injection 
Proptosis 
Globe displacement 
Myositis if EOMs involved. 
Dacryoadenitis

A

Compression on the globe and/or ON

Management

A

Orbital CT scan/MRI to rule out orbital cellulitis or tumor.
Oral steroids. Responds well!!!

A

Very quick, painful onset. Present in 65% of cases
