A middle aged man presents with joint pains, anorexia, diarrhoea, and intermittent fever. He has lost 5kg over 6 months, and currently has abdominal distension, inguinal lymphadenopathy, and pitting bipedal oedema. Lab investigations show no abnormality however jejunal biopsy shows expanded villi and period acid-Schiff-positive macrophages. What is the diagnosis?
Whipple’s disease.
32 year old Thai woman presents to A&E with a 4-month history of hypoaesthetic erythematous plaque-like lesions/rash on her arms and elbows asking for a refill for medication she has been on for the past 4 weeks. She says she has had needle tests on her ear and elbow which were “positive” as well as a skin biopsy. Routine bloods normal. Apyrexial. What is the most appropriate treatment?
Dapsone, clofazimine, and rifampicin.
