Integumentary Week 2 - unfinished Flashcards
(27 cards)
Dressings have four main jobs:
- Protect and insulate wound
- Keep wound bed moist but also prevent periwound from being macerated
- Fill voids in the wound to promote bottom healing up
- Decrease bioburden or bacterial load
Primary Dressing definition
comes in direct contact with the wound
Secondary Dressing definition
placed over a primary dressing to provide increased protection, cushioning, absorption or occlusion
Advantages of using Gauze over a wound
Ease of use
Cheap
Versatile, can be used with lots of materials
When do you use gauze?
-scrubbing/prepping
-absorption,
-protection,
-filling space
-delivery mechanism for solutions
T/F Gauze is woven only
False; it can be woven or non-woven
Semipermeable/Semi-occlusive Film contains a ________ ________ layer, and allows ……
porous adhesive layer;
allows transmission of O2 & moisture vapor
6 Advantages of Semipermeable/Semi-occlusive Film
-wound visualization
-impermeable to fluids & bacteria
-conformable
-autolytic debridement
-reduces friction
-change daily or up to weekly (7 days)
3 Disadvantages of Semipermeable/Semi-occlusive Film
-does not absorb
-adherent to dry skin
-may cause maceration
What dressing would you use:
with a stage I or II PU;
as a primary or secondary dressing; or on low exudating wounds?
Semipermeable/Semi-occlusive Film
Hydrogels - Amorphous are made of …….
Non cross-linked polymers plus water &/or glycerin
-can contain other ingredients like silver or alginate
4 Advantages of Hydrogels-Amorphous
-hydration of wound bed
-autolytic debridement
-easily cleaned
-may reduce pain
4 Disadvantages of Hydrogels-Amorphous
-may macerate
-minimal absorption,
-requires secondary dressing
-daily dressing changes usually required
What dressing would you use with a:
Stage II, III, or IV PU;
abrasion/skin tear?
Hydrogels-Amorphous
OR
Hydrogel-Sheets
Hydrogel-Sheets are made of ……
Cross-linked polymers with central mesh to hold them together
aka, made of water & glycerin
7 Advantages of Hydrogel-Sheets
-hydration of wound bed
-autolytic debridement
-some absorption
-non-adherent
-may have self adherent border
-may stay in place 2-3 days
-may reduce pain
4 Disadvantages of Hydrogel-Sheets
-potential for maceration
-minimal absorption
-requires secondary dressing (if not bordered)
-only minimally fill empty spaces
Foam is made of …..
Polyurethane open cell sheets, can be single or multilayered
4 Advantages of Foam
-highly absorptive
-can be used when infected
-available with or without border
-can be used in tunnel or undermining
3 Disadvantages of Foam
-may require additional tape
-may macerate if hydration extends beyond border of wound
-not recommended for dry eschar
What dressing would you use with a:
Stage II, III, IV PI with minimal to heavy exudate,
tunnels or cavities
Foam
Hydrocolloids are made of …..
Hydrophyllic colloid particles bound to polyurethene foam or film
Hydrocolloids form _____ on contact with _______ _______ and are __________ to _________
form gel on contact with wound fluid
impermeable to bacteria
9 Advantages of Hydrocolloids
-moist environment,
-impermeable to bacteria & moisture,
-min to mod absorption,
-autolytic debridement,
-thermal insulation,
-wear up to 7 days,
-self-adhering,
-many shapes/sizes,
-available with silver
