What legally has to be on the dispensed medical product?
- Name of patient
- Name and address of pharmacy
- Date of dispensing
- Name of medicine
- Directions for use
- Precautions - eg. For external use only
What is recommended to appear on the dispensed medicinal product?
- Keep out of reach and sight of children
- Use only on your skin - where applicable
- If for a prisoner - strongly rec the prisoner number is included on the label as a definitive patient identifier
Where should the label be placed?
Actual container not the box as it could be discarded
If medicines are dispensed into skillets from bulk what must be on the label?
- Name of the medicine
- Quantity in container
- Quantitative particulars of the medicine - ingredients
- Handling and storage requirements
- Expiry date
- Batch reference number
What must appear on the dispensing label for a veterinary med prescribed under the cascade?
- Name of prescribing vet surgeon
- Name and address of animal owner
- Name and address of pharmacy
- Identification and species of animal
- Date of supply
- Expiry date
- Name/description of product or its active ingredients and content quantity
- Dosage and administration instructions
- Special storage instructions if appropriate
- Any necessary warnings for the user
- admin, disposal, target species etc.
- Any applicable withdrawal period
- ‘For animal treatment only’
- ‘Keep out of reach of children’
Is a dispensing label required if the medicine is not prescribed under the cascade?
VMR say not req but should be done
