What special things to know about the antidepressants?
Wellbutrin, Zyban. Cymbalta. Remeron. Pamelor. Buproprion inhibits the reuptake of norep and dopa. Dulox is sero and nor and kind of dopa. Mirtaz is alpha 2 adre antag. Nortrip is Sero and Nor(tri cyclic).
Buproprion is 100 mg BID and XR is 150 mg once daily. Duloxetine is 30-60 mg in a single dose or 2 divided doses. Mirtazepine is 15-45 mg qhs. Nortriptyline is 25 mg TID or QID, may titrate to 150 mg/day.
BBW is suicide, Watch for history of anorexia, MAOI use in 14 days, seizures, insomnia, aggitaiton, weight loss. USe with linezolid, headache, drowsiness. Heart arrythmias in Nortriptyline.

Bentyl(used for IBS), Vesicare(overactive bladder). Inhibits acetylcholine.
Capsule, solution, tablet, gel, IM. 20-40 mg QID for Dicyclomine. 5 mg once daily, may increase to 10 for Solifenacin.
CI’d in obstructive GI, breast feeding, digestive problems. Watch for Dizziness, Blurred vision, xerostomia, nausea. Solifenacin CI’d in urinary and gastric retention and narrow angle glaucoma. Drink plenty of water and don’t crush or chew.

Diflucan, interfers with p450 and ergosterol. Comes as IV, solution, tablet. For candidas give 150 mg once and candidemia is 800 mg day one and 400 mg daily after. CI’d in cyp3a4 administration due to QT prolongation. Watch for HA, N/V, hepatitis and jaundice.
Neurontin. Affects gaba neurotransmitters. Capsule,tablet, solution, suspension, cream. 900-1800 mg daily in 3 doses. CI’d in hypersensitivity, watch for drowsiness, dizziness, fatigue, tremor.
Synthroid,Tirosint, Unithroid, Levoxyl. Thyrolar. Cytomel. All affect thyroid, T4 is synthroid and T3 is Cytomel. Tablet, Capsule, IV–> 25-300 mcg once daily. BBW is weight reduction, CI’d in acute MI, renal problems, thyroidtoxicis. Take on empty stomach, have labs checked.

Provera, Depo-provera. Inhibits secretion of pituitary gonadotropins. Comes as tablet, subq, IM. 5-10 mg once daily or 150 mg IM every 3 months. Lots of BBW and side effects.
Dolophine, Methadose. Opiates and NMDA receptors. tablet, solution, concentrate, injection 2.5 q8-12h. Detox is 80-120 mg daily. Lots of stuff.
Namenda, NMDA antagonist. Tablet, Capsule(XR), Solution. IR: 5-20 mg daily. XR: 7-28 mg once daily.

Concerta, Daytrana, Ritelin, Methadate. Blocks norepinephrine and dopamine reuptake. BBW abuse and dependence, CI’d in MAOI use or marked anxiety or irritation. Watch for Insomnia, weight loss, tachycardia, hypertension. Dosing 10-60 BID or TID. ER: is 20 mg once daily.
Zyprexa. Antagonist of dopamine and seratonin. Tablet, Disintegrating, IM 5-20 mg once daily. BBW is increased mortality, post injection delirium. Watch for increased weight gain, glucose, prolactin levels, drowsiness.
Zofran, Zuplenz. 5-HT3 receptor antagonist. Watch for headache, mailaise, fatigue, constipation. CI’d in Hypersensitivity or use with apomorphine. Injection, film, solution(injection). 8-16 mg 30 min to 1 hr before therapy.

Ditropan, Oxytrol. Inhibits acetylcholine, antisposmadic effect. Tablet, Gel, Patch, Syrup 5-10 mg once daily. CI’d in narrow angle glaucoma and anti cholinergic effects. Watch for dizziness.
Deltasone. 5-60 mg once daily. You know the rest from your test.
Harvoni. BBW is hep b reactivation. Watch for Hyperkalemia, Gynacomastia, monitor renal function. Oral tablet 400 mg 400 mg/9mg once daily.

Androgel, Androderm, Testim, Axiron, Striant. IM, Solution, implant, nasal, transdermal, buccal. ms Gel: 50mg q AM (Max 100mg/day) Patch: 1 patch once daily INJ: 50mg to 400mg IM q 2 to 4 weeks. BBW of secondary exposure, pulmonary oil microembolism, watch for Edema Hypertension CHF Erythrocytosis and don’t give to women.
Topamax, Works at GABA, blocks sodium channels. CI’d in recent alcohol use for XR formulation. Watch for mental problems, weight loss, kidney stones. Capsule, sprinkle, Tablet, 25-200 mg BID.
Ultram, blocks opioid receptors. Cream, Suspension, tablet, capsule, 50-100 mg once q4h, or 100 mg once daily. You know it all. Dose in renal adjustment/elderly and watch for seizure risk.
