A

Fosamax, Actonel. Inhibits bone resorption via actions on osteoclasts or osteoclast precursor.
Prophylaxis: 5mg once daily or 35 mg once weekly, Treatment is 10 mg once daily or 70 mg once weekly.
Females: 5 mg once daily or 35 mg once weekly or 150mg once monthly. Males: 35 mg once weekly.

CI’d in hypocalcemia, esophageal abnormalities, inability to sit or stand for 30 minutes. Must take on empty stomach before breakfast with full glass of water, do not eat drink or take other meds and remain upright 30 minutes post dose, notify dentist you take this due to rare osteonecrosis of jaw. Maintain vitamin D and calcium supplementation, bone pain.

Nexium, Prevacid, Prilosec, Protonix, Aciphex. Suppresses gastric acid secretion by inhibition of the H+/K+- ATPase in the gastric parietal cell. CI’d in hypersensitivity, can cause headache, diarrhea, and need to take 60 minutes before a meal. Omeprazole may diminish the effect of clopidogrel.

A

Comes IV, capsule, Packet, 20-40 mg once daily. Comes capsule, suspension, disintegrating tablet 15-30 mg once daily or for Hpylori it’s 30 mg BID or TID. 20-40 mg once daily(tablet, capsule, solution, packet).

40 mg once daily or BID(tablet, packet, IV). 20 mg once daily or Hpylori 20 mg BID(tablet, capsule).

Synflorix, Prevnar 13. Pneumovax 23. Varivax. 0.5 mL single dose. Varicella is 0.5 mL twice seperated by 28 days.

CI’d in diptheria toxoid allergic reaction(prevnar 13) or pneuemococcal reaction(pneumovax 23), Varicella CI’d in Hypersensitivity to the vaccine, neomycin, or gelatin
Immunosuppressed/deficient individuals Active, untreated TB or other current febrile illness ,Pregnancy. Watch for Chills, Skin Rash, Local Erythema, Arthralgia, injection site reaction, fever.

A

Chantix, Partial neuronal α4 β2 nicotinic receptor agonist. tablet onlye 0.5 mg once daily for 3 days, 0.5 mg BID for 4 days, 1 mg BID for 11 weeks.

CI’d in hypersensitivity, Watch for HA, N&V, insomnia, abnormal dreams, suicidal idiation.

Moxatag, binds to penicillin binding proteins which inhibits peptidoglycan synthesis. 500mg q8hours or 875mg q12hours.

Serious hypersensitivity to amoxicillin, Diarrhea, Allergy, take entire course and contact provider if you don’t improve in a few days. N/V, take with food to avoid upset stomach if needed.

A

Augmentin, MOA is same except Clavulanic acid binds to beta lactamases that inhibit Amoxicillin. All the same as Amoxicillin.

Bicillin L-A, Same as penicillin for special considerations and MOA, Comes as Suspension IM 1.2 million to 2.4 million units IM.

Apo-Pen, NovoPen, NuPen VK used for respiratory infections, interferes with bacterial cell wall synthesis, 125mg to 500mg q 6 to 8 hours(tablet and solution reconstituted).

Pyridium, Unknown(local urinary tract anesthetic),200mg TID after meals(95,100,200 mg tablets), CI’d in hypersensitivity and renal insufficiency and do not exceed 48-72 hours, changes urine color.

A

Pepcid, Zantac, Phenadoz, Phenergan, Promethagan. Completely inhibits H2 receptors at the parietal cells,Blocks postsynaptic mesolimbic dopaminergic receptors in the brain; exhibits a strong alpha adrenergic blocking effect and depresses the release of hypothalamic and hypophyseal hormones; competes with histamine for the H1-receptor.

20-40 mg once daily or BID(comes IV or tablet). 150 mg BID or 300 mg QHS(comes as syrup and injection). 12.5mg to 25mg po or PR q HS or q 4 to 6 hours PRN,25mg po or PR 30min to 1 hour before departure

CI’d in hypersensitivity, watch for headache and diarrhea. CI’d in hypersensitivity, children under 2, treatment of lower respiratory tract infections including asthma, watch for sedation and possible EKG changes.

You know what to watch for.

Ambien, Enhances activity of GABA via selective agonism at the benzodiazepine-1 (BZ1) receptor. IR: 5mg (female) or 5mg to 10 mg (male) q HS ER: 6.25mg (female) or 6.25mg to 12.5mg (male) q HS.

CI’d in hypersensitivity, Watch for drowsiness/sedation, lethargy, and memory impairment.

A

Flomax. No. Antagonist of alpha1A-adrenoreceptors leading to relaxation of smooth muscle. 0.4 mg capsule once daily, 1-10 mg qhs(max 20mg/day).

CI’d in hypersensitivity. Don’t crush/chew , take on an empty stomach, watch for tiredness, dizziness, orthostatic hypotension, take terazosin at bedtime.

Imitrex, Sumavel. Selective agonist for serotonin on intracranial blood vessels and sensory nerves of the trigeminal system. Tablet, Nasal solution and powder, subcutaneous, transdermal. 25-100 mg sindle dose at onset.

CI’d in Ischemic heart disease, uncotrolledhypertension, PVD, or cerebrovascular syndromes MAOI use within 14 days Use of ergotamine derivatives within 24 hours
Severe hepatic impairment. Watch for Parasthesia, Dizziness, Flushing and risk of seratonin syndrome.
