16
Q

infantile idiopathic scoliosis occurs in children __yo

17
Q

what is treatment for infantile idiopathic scoliosis if mild? Bracing and exercise is recommended at __-__ degrees. __ degrees is a surgical referral

A

mild - may resolve on its own
25-40 = bracing and exercise
>40 = surgical referral

18
Q

what are the anomalies of infantile idiopathic scoliosis?

A

VACTERL syndrome =
- Vertebral anomalies
- Anal atresia
- Cardiac defects
- Tracheo-esophageal fistula
- Renal anomalies
- Limb defects

19
Q

What is a hemivertebra?

A

failure of formation, segmentation or both. Half of the somite forms resulting in congenital scoliosis

20
Q

what are the different types and locations of hemivertbra?

A

lateral, dorsal or ventral (rare)

fully segmented, semi-segmented, non-segmented, incarcerated

21
Q

When is hemivertbra diagnosed?

A

can be first few days of life to 25 years (average is 6yo)

22
Q

What is the treatment for hemivertbra?

A

if neurological deficits, consider MRI (tethered cord, syrinx, chiari, diastematomyelia)

bracing (not the best)

surgery (difficult)

23
Q

What is butterfly vertebra?

A

failure of fusion of the lateral halves of the vertebral body

persistent notochordal tissue between them

24
Q

What is the radiographic appearance of butterfly vertebra?

A
  • interpedicle diastisis
  • bodies above and below have altered shape
  • may have bony bridging

can be mistaken for burst fracture

25
how common is rib synostosis? symptoms?
relatively common (3% of population) generally asymptomatic, may result in scoliosis or Tx insufficiency can be unilateral or bilateral
26
how is a bifid rib discovered? how common is it?
incidental finding, may be palpated as a chest “lump” 0.15-3.4% of population (20% of rib anomalies) M>F
27
How common is pectus excavatum? M:F? cause?
1 in 300 live births M:F 3:1 due to overgrowth of costal cartilages that rotate and curve dorsally
28
what are the radiographic features of pectus excavatum?
- blurring of right heart border - horizontal posterior ribs - vertical anterior ribs (heart-shaped) - displacement of heart towards the left - apparent widening of cardiac silhouette
29
what is the treatment for pectus excavatum?
generally nothing check respiratory function mental health factors
30
what are the symptoms and treatments for pectus carinatum?
generally asymptomatic - may have dyspnea or exercise intolerance bracing or surgery is symptomatic
