What are the treatment options of post-herpetic neuralgia?
- Gabapentin, pregabalin
- Lidocaine
- Carbamazepine, Oxcarbamazepine (not FDA approved but useful)
What vessels are classically associated with epidural hematoma?
Subdural hematoma?
Epidural hematoma = Middle meningeal Artery.
Subdural hematoma = Bridging veins.
Creutzfeldt-Jakob disease presents with what EEG pattern?
Classically theta and delta wave with PERIODIC BURST-SUPPRESSION PATTERN and PERIODIC PAROXYSMAL SHARP WAVE COMPLEX.
What do Mee’s lines on fingernail suggest? (white transverse lines on fingernail bed.)
Arsenic poisoning (usually occurs 3-6 weeks post exposure)
What is the mechanism of organophosphate poisoning?
Inhibition of Acetylcholinesterase (by phosphorylation) causing cholinergic toxicity.
What are the symptoms of organophosphate poisoning.
DUMBELSS
Diarrhea Urination Meiosis Bronchospasm Emesis Lacrimation Salivation Sweating
How do you treat organophosphate poisoning?
Atropine OR Pralidoxime
How do children and Adults differ in their presentation of LEAD poisoning?
Children = Acute GI illness then Acute encephalopathy, with confusion, lethargy, seizures, Intracranial HTN.
Adults = Polyneuropathy (classically motor predominant with wrist drop and foot drop)
+/- blue gingival line +/- hypochromatic microcytic anemia
What is the treatment of Lead poisoning?
Dimercaptal (chelation) if levels high enough.
