Combination of Absolute Neutrophil Count (ANC) <500 and Temperature >100.4F

What tests are used to diagnose Neutropenic Fever?
A
Blood Cx, UA and CXR

How do you treat Neutropenic Fever?
A
Start with one of the following abx
-Cefepime
-Meropenem or Imipenem
-Piperacillin/tazobactam

A
Cefepime
Meropenem (or Imipenem)
Piperacillin/Tazobactam

If a patient is still febrile after 2-3 days of initial treatment what is the next step?
A
Add vancomycin

If a patient remains febrile 2-3 days after the addition of Vancomycin what is the next step?
A
Add an antifungal: Echinocandin (Micafungin) or Voriconazole (-azole that covers aspergillus)
