What is the definition of Gout?
A metabolic disorder characterized by disruption of purine metabolism, manifested by hyperuricemia (> 7 mg/dL), deposition of urate crystals in joints (arthritis), kidney (nephropathy), and under the skin (Tophi).
What is the primary manifestation of hyperuricemia in Gout?
Hyperuricemia > 7 mg/dL.
What are the three main treatments for an acute attack of Gout?
- Colchicine
- NSAIDs
- Steroids
What is the role of Colchicine in treating acute Gout attacks?
Effective for treatment of acute attack but less favored due to GI side effects and slow onset.
Which drugs are considered the treatment of choice for acute Gout attacks?
NSAIDs, e.g., naproxen and indomethacin.
When are steroids used in Gout treatment?
Reserved for patients unable to take NSAIDs, e.g., ACTH or prednisolone.
What is the action of Colchicine?
Interferes with phagocytosis and lactic acid production, leading to precipitation of uric acid and no acidosis.
Name a drug contraindicated in Gout treatment.
- Diuretics (Thiazide & loop)
- Salicylates with small dose < 5 g/day
- Probenecid with small dose < 1 g/day
- Diazoxide (D.V.D)
What is the goal of prophylaxis in Gout treatment?
To decrease blood levels of uric acid (< 6 mg/100 ml blood).
What type of drugs are used for uricosuric prophylaxis in Gout?
- Salicylate in large dose > 5 g/day
- Probenecid in large dose > 1 g/day
- Benzbromarone 40-80 mg/day
What is the mechanism of action of Allopurinol?
Inhibits enzyme xanthine oxidase, preventing uric acid synthesis from xanthine and hypoxanthine.
What conditions is Allopurinol used for?
- Hyperuricemia
- Chronic gout
- Gout nephropathy (impaired renal functions)
- Urate renal stones
