patient assessment Flashcards
(27 cards)
when i assess the scene for safety i am:
determining the need for PPE (personal protective equipment)/BSI (body substance isolation) and assessing personnel and patient safety
after assessing for safety i:
determine the need for additional resources. (ALS, firefighters, etc)
the first step in my primary assessment is to:
formulate a general impression of the patient - their position, the environment, am i assuming a spinal injury? is there a massive hemorrhage?
second step in my primary assessment is:
to establish a patient rapport - introduce myself, address their chief complaint
third step in my primary assessment is:
assess their mental status
AVPU stands for?
Alert, Verbal, Pain, Unresponsive
the four questions used to orient a patient are?
Person, Time, Location, Event
fourth step in my primary assessment is the “A” in ABC
assess the airway - is it patent or obstructed? - manage the airway
fifth step in my primary assessment is the “B” in ABC
Assess their breathing - adequate or inadequate breathing? - oxygen administration
sixth step in my primary assessment is the “C” in ABC
assess their circulation - take pulse - observe skin color and condition - obvious external bleeding?
the final decision in my primary assessment is?
the transport decision - are they critical or noncritical?
if the patient in unresponsive/unconscious i skip to what step in ABC?
C! check circulation. do they have a pulse?
the first step in my secondary assessment is:
assessment of the chief compliant - OPQRST & SAMPLE
OPQRST?
Onset, Provocation, Quality, Radiate, Severity, Time
SAMPLE?
Signs/Symptoms, Allergies, Medications, Pertinent history, Last oral intake, Event
during the secondary assessment we do a systematic assessment of:
medical issues or trauma issues. as determined by the scene
during the secondary assessment we do a full rundown of:
vital signs
key vitals to take during a secondary assessment:
BP, pulse, respiration rate, temperature, SpO2, lung sounds
at the very end of the secondary assessment we look into:
additional diagnostics - glucose reading, pupils, ETCO2 (end tidal carbon dioxide), GCS (glasgow coma scale)
PMS?
Pulse, Motor, Sensation
The two questions and one vital to assess during PMS?
Can you feel this? Can you wiggle your toes/fingers? take pulse
If a patient is critical retake their vitals every…
5 minutes
If a patient is NONcritical retake their vitals every…
15 minutes
the treatment and reassessment stage of patient assessment has four parts. they are:
your field impression, your treatment plan, your timely identification of clinical changes, and your reassessment during transport
