When to discontinue Analgesia?
NSAIDS and cox-2 inhibitors: 7 days before surgery
When to stop anticoagulation?
Before major surgery
When to stop Antiplatelet?
Noncardiac patients before major surgery
When to stop cardiovascular drugs?
continue BB and CCB
Stop Diuretics day of surgery
When to stop Estrogen?
Several weeks before due to increased DVT risk
When to stop GI medications?
Continue H2 and PPI
When to stop Glucocorticoids?
continue steroids
When to stop Herbal meds?
stop 1 week before
When to stop Hypoglycemic agents?
Oral: 3 days before
Short insulin: withhold on morning of
Long insulin: continue at half-dose
When to stop Immunomodulators?
Nontransplant: 2 weeks befor
Transplant: continue all except Sirolimus (increases poorhealing/dehiscence)
When to stop lipid lowering meds?
Withhold day of surgery
When to stop Neurologic meds?
Continue antiepileptics
Stop dementia drugs before
When to stop Pulmonary meds?
Continue all inhalers and glucocorticoids
When to stop Thyroid medications?
Continue thyroid meds
