What is Post-op fever days 1-2 associated with?
Wind; Atelectasis or Pneumonia
What test is used for POD1-2 fever?
CXR followed by sputum cx
What therapy is used for POD1-2 fever?
Prevention with incentive spirometry
Abx for HAP
What is POD3-5 fever associated with?
Water: UTI
What test is used for POD3-5 fever?
UA showing (+) Nitrates and Leukocyte esterase
Ucx for species and sensitivity
What therapy is used for POD3-5 fever?
Abx
What is POD5-7 fever associated with?
Walking: DVT, Thrombophlebitis of IV access, PE for new-onset tachycardia and chest pain
What tests are used for POD5-7 Fever ?
Doppler US of the extremities, change IV lines, Cx iv tips
What therapy is used for POD5-7 fever?
DOAC for clot
What is POD7 fever associated with?
Wound: wound infection, cellulitis
What tests are used for POD7 fever?
Physical exam for wound erythema, purulent discharge and or swelling
What therapy is used for POD7 fever?
I&D if abscess of fluid followed by Abx
What is POD8-15 Fever associated with?
Wonder: drug fever or deep abscess
What tests are used for POD8-15 Fever?
CT to examine for deep fluid collection
What therapies are used for POD8-15 Fever?
CT guided percutaneous drainage of the abscess: otherwise surgery
What are common causes of post-op confusion?
Hypoxia or septic
What should you order for a patient with post-op confusion?
ABG, CXR, blood Cx, urine Cx, CBC
What is Superficial Wound Dehiscence?
Skin and tissues have separated but rectus fascia is intact
How do you treat superficial wound dehiscence?
Careful dressing changes
What is Deep wound dehiscence?
Involves the rectus fascia exposing the abdominal organs to external air
How do you manage Deep wound dihiscence?
emergency surgery and Abx
How does Acute Respiratory Distress Syndrome (ARDS) present?
severe hypoxia, tachypnea, accessory muscle use and hypercapnia
How do you diagnose ARDS?
CXR; bilateral pulmonary infiltrates without JVD
How do you manage ARDS?
Positive end expiratory pressure (PEEP)
