What constitutes tardiness of 30 minutes or more?
Failure to report
District Chief will be notified and charges filed with Fire Chief.

For 40-hour personnel, 3 days missed requires a doctor’s release.
What happens if an individual has more than 3 occurrences of sick leave in 120 days for 51-hour personnel?
May be required to see a doctor
Department will cover medical expenses.
What is the requirement for an employee with more than 4 occurrences of sick leave in a calendar year?
Must see a doctor and present an explanation of illness
What is considered extended sick leave?
More than 3 duty shifts
Employee must report to respective D/C office by 0900 on specified days.
What form must be presented upon returning from extended sick leave?
Completed #517A, signed by a doctor
Who must the completed #517A form be turned into before returning to duty?
D/C or A-1
For whom can employees use sick leave due to illness or medical appointments?
- Child/step-child
- Spouse/domestic partner
- Sibling
- Parent
- Mother-in-law/Father-in-law
- Grandchild
- Grandparent/Step Parent
What information is required when reporting absence for family medical reasons?
- Name of family member
- General reason for absence
- Required length of absence
When should notification of absence be made?
As soon as possible, generally at least 1 duty day prior
Written verification may be required.

144 hours/year
What are the benefit limits for sick leave for 40-hour employees?
48 hours/year
