Etiology:
A
- Rickettsia Rickettsiae → RMSF ; common in North &
south USA - Rickettsia Conorii → MSF = Mediterranean spotted
Fever; common in Israel

route of Transmission
A
contact with Dog [Dog bite /Tick bite]

Clinically
:
A
The patient will come with Flu like symptoms [ Fever,
malaise , headache , myalgia ] Then Maculopapular
RASH starting in palm & soles and then progress to
trunk & rest of body areas

lab
A
anemia,thrombocytopenia,
CRP HIGH,ESR high ,ALT,AST high
low albumin ,hyponatremia

Diagnosis
A
- usually it diagnosed based on the clinical features, but confirmation done by in
one of 4 ways:
1. Serology [positive only after 7- 10 day from exposure]
2. Also, Biopsy from skin rash or eschar
3. PCR for rickettsia DNA
4. Isolation of rickettsia

Treatment
A
Doxycycline REGARDLESS the age!! [not as Lyme disease]
unless pregnant woman → Chloramphenicol

the adverse effect of chloramphenicol if used in neonate?
A
Grey baby syndrome
