What is SFBT
- Goal driven approach
- Focus on clients health and well being and their strengths rather than the problem.
- Nothing happens 24/7 focus on the exceptions
What can SFBT be used for ?
- long term health conditions
- Pain management
- Family dysfunction
- weight management
- self esteem problems
- stopping smoking
- drug and alcohol abuse
What are the elements of SFBT
- people are free to make choices
- they have the resources to solve their problem
- they have copped with other problems in the past
- the client is the expert
What did de shazer and berg find with their clients ?
Clients felt empowered as they described what they wanted in their lives.
Clients are encouraged to develop a vision of an alternative future and believe that change is always possible.
What are the 5 steps of SFBT
2 - separating the person from the problem 3- miracle question 4- exceptions 5- scaling questions 6- goals/feedback
Rules of SBFT
1 - If it isn’t broken don’t fix it.
2- If it works, do more of it.
3- If its not working do something different
Role of the nurse
- be curious
- ask questions as part of a conversation not as a tick list
- constructive questions to determine clients own strengths and capabilities.
- generate problem free talk - helps to develop the relationship and focus on the positives
Name an example of a positive solution focused question.
- what would you be doing if you did not have this problem ?
- Are there times where the problem seems less ? Tell me about them
How should goals be set (SMART)
Specific
Measurable - help you evaluate achievement and progress
Attainable - The goal should be challenging but still reasonable to achieve.
Relevant - ensure what you are trying to achieve is worth while to you
Time bound - every goal needs a target date to motivate the person
Explain the miracle question
- suppose you went to bed tonight, a miracle happens and your problem is solved but you don’t know it because you were asleep. What will the following day be like when the problem is solved ?
- Allows patients to think about what will be present in their life when the problem is solved.
- People get stuck in their life, the miracle question let’s the person experience what their life COULD be like.
Explain the exception process
- There are always exceptions to any problem it’s never 24/7
- when doesn’t the problem happen ?
- what’s different about them times ?
- What are you doing differently during good times ?
What are some coping questions for the current problem ?
- how do you cope with this ?
- what keeps you going ?
- who is your greatest support, what do they do that is helpful ?
- sometimes problems get worse, what do you do to stop it getting worse?
What are some coping questions for a past problem ?
- how did you get through this ?
- who was your greatest support ?
- how did they help ?
- how did you manage to solve that problem in the past ?
Explain scaling questions
- scale (1-10) 10 being the worst pain
- if the patient says 5 ask what would you be doing differently if it was 4 ?
- what have you done to get to 4 ?
- who would be the first person to notice you get to 3 ?
