Scene Size-up Steps

A
  1. BSI, Scene safety
  2. Determine mechanism of injury and how many patients, if other resources are needed and call for ALS
  3. Ask trained assistant to manually stabilize the patients C-spine
  4. Check for any stomas, medical alert tags, deviated tracheas and JVD (Jugular Vein Distention

Primary Assessment Step

A
  1. Give a general impression and determine chief complaint
  2. Determine if the patient is alert and oriented
  3. Airway- have trained assistant perform a jaw-thrust maneuver so an OPA can be inserted to maintain a clear airway on the patient. Give the patient oxygen with a non-rebreather mask set to 12-15 liters of oxygen. Check neck to make sure there isn’t anything causing obstructing the airway
  4. Breathing- Check lung sounds in 4 spots (upper r&l shoulder and right under r&l ribs), check for. equal lung expansion by placing both hands under the pts ribs, compare your breathing to the patients breathing by placing your hand on their abdomen and place your hand on your abdomen and compare breathing, expose the pts chest to see if there is any penetrating trauma
  5. Circulation- Check for the patients pulse, check the patients skin for color, temperature, and condition, and check to see if there is any blood in the voids
  6. Perform a rapid head to toe assessment of the patient
  7. Decide that the patient is going to be a rapid transport, update your crew about the patients condition and identify the priority of the patient

History and Secondary Assessment Step

  1. Due to the patient being unconscious unable to obtain the patients SAMPLE (Signs and symptoms, Allergies, Medications, Past medical history, Last oral intake, and Events leading up to event)
  2. Perform full body assessment checking all extremities (check pulse on all 4 limbs), check head for any abrasions lacerations, check face for any abrasions lacerations any airway obstruction, check the patients pupils with a pen light to see if they are equal and reactive, recheck JDV and trachea position, check ears for battle signs and if there is any blood or fluids present, check nose to see if any blood or fluids present, palpate the shoulder chest area, recheck the patient’s lung sounds, palpate and push around the patients abdomen to check for any distention or deformities, check both arms and legs for any lacerations, contusions, abrasions.
  3. Assess the patient’s hips press in and down to make sure there isn’t any dislocations or fractures as well as checking for any fluid or blood (females) or an erection (males)
  4. Log roll the patient over onto their side to check the patients spine, back of head, back of legs, back of arms, and butt for any deformities, lacerations abrasions. Have trained assistant place a back board underneath the patient to help with getting the patient onto the stretcher.
  5. ASK for vital signs: What is the patient’s blood pressure?, What is the patients pulse rate and quality?, What is the patients respirations rate and quality?. Redo skin CTC (color, temp., condition)
  6. Update my crew view radio about the patients condition

Reassessment Steps

A
  1. Update my crew about the patient
  2. Make sure C-Spine is still stabilized
  3. Repeat ABC’s
  4. Repeat the patient’s vitals every 5 minutes
  5. Rapid transport the patient to the hospital
