Accumulation of fluid in tunica vaginalis

  • communicating - common in newborn males
  • non communicating - excess fluid prod within tunica vaginalis

Presentation

  • soft, non tender swelling confined to scrotum
  • transillumates
  • difficult to palpate testes

Diagnosis - clinical but can use US if in doubt

Management

  • infants - surgical repair if no resolution in 2 years
  • adults - conservative

Epididymal cysts

Most common fluid filled benign swelling
Associated with PKD, CF, von HippelLindau

Separate from body of testicle

Diagnosis confirmed with US

Management - supportive
-surgical removal for larger, symptomatic cysts

Normally aymptomatic but can cause infertility

More common on left
Bag of worms on standing
Infertility

Diagnosis - Doppler studies

Management - conservative
-surgery if causing pain
