What is the goal of a fishbone diagram?
Used for grouping causal factors in a Root Cause Analysis
What is DMAIC?
Steps for a Root Cause Analysis coupled with an action plan: Define, Measure, Analyze, Improve, Control
What are the various categories of probability?
- Frequent: at least monthly
- Occasional: 1-2x / yr
- Uncommon: 2-5 yrs
- Remote: <5 yrs
What are some hallmarks of a MAJOR event?
Incident leads to hospitalization. Fall with injury which impacts fx. Theft/Fire/Significant equipment damage.
What are some hallmarks of a MODERATE event?
Potentially avoidable complication of an existing condition (i.e. wound becomes infected, UTI while using catheter).
What are some hallmarks of a SENTINEL event?
Unexpected patient death.
Injury/incident that could lead to permanent change in fx.
What is strategic analytics?
Provides the right info at the right time to enable leaders to make informed biz decisions.
What is the difference between a single incident response and an individual performance improvement?
Something bad happened vs. an employee requires behavioral change
What are the main functions of a QAPI?
Quality Assurance and Performance Improvement: focuses on indicators related to improved outcomes and take actions to address the performance across the spectrum of care. It also maintains documentation of all of this.
Examples: RCA, Analytics and Data Reporting, DMAIC or PDSA, Evidence of Employee-level Performance Improvement Planning
How do you change employee behavior?
- Set clear expectations / WHY?
- Verify competency and schedule times to revisit progress
- Hold people accountable
