Week 11 (part 1 and 2) Flashcards
(7 cards)
part 1
practical exam preparation
how do you test sensation for upper and lower limbs
Sharp/ dull: - eyes closed, see if they can accurately say whether the neuro tip is sharp or dull
can be done on hands and feet as well
Cotton wool (dermatomes) - does this feel the same as that? eyes closed checking sensation on either side of body
Others?
how do you test and improve lower limb power
functional exercises like squats and jumps - with speed otherwise could be considered as strength exercises
TESTING**
how do you test and train upper and lower limb tone or proprioception
Testing:
spatial awareness - moving upper limb in certain position and getting them to copy, with eyes closed
Up/down - moving fingers or toes up or down and checking the patient knows whether the limb has been moved up or down HOLD LIMB ON THE SIDES (so patient cant feel pressure from hand)
Training?
how do you test and train strength
TESTING:
isometric strength testing using resistance (patient and physios positioning, plinth height etc)
Oxford grading 0-5
0 - no muscle movement
1 - muscle twitch
2 - full rom without gravity
3 - full rom against gravity
4 - muscle contraction with minimal isometric resistance
5 - muscle contraction against full resistance from physio
TRAINING:
Functional activities like STS - reps and sets to improve dynamic strength and strength endurance
Isometric holds to improve isometric strength
how do you test and train balance in sitting and standing
TESTING:
talk about LOG, BOS and COM, when patient is unbalanced
sitting - push the patient from 360 degree positions on the upper body to test core strength and trunk muscles
sit and reach - get patient to reach and touch your hand, moving hand around to different distances and positions
standing - ^^ do the same but in standing
TRAINING:
Single leg stand - reduces BOS, makes balancing harder
*****
explain the pathology of MS and how it presents
