What can cause specific low back pain?
Ankylosing spondylitis, spinal stenosis, spondylosis, spondylolysis, spondylolisthesis, scoliosis, whiplash
What is ankylosing spondylitis?
-inflammatory arthritis, mainly affects entheses and where the ligaments, tendons and capsules attached to the bone
What is typical presentation for ankylosing spondylitis?
-onset of back pain and stiffness
-weight loss
-fatigue or tiredness, feeling feverish and experiencing night sweats
What is spinal stenosis?
-canal narrowing, either central or foraminal, osteophytes, disc herniation, thickening of ligament, tumours
-central cord compression in cervical can cause myelopathy
-central compression at the bottom can cause cauda equine syndrome.
What is the management for spinal stenosis?
-surgery, advice and education, pain management, exercise, lifestyle changes
What is spondylosis?
-Generic umbrella term for osteoarthritic changes within the spine
-may cause stenosis, osteophyte formation
What is the management for spondylosis?
-reassurance, encourage normal movement and function, increase ROM, increase strength, graded exposure
What is spondylolisthesis?
-Anterior translation of the superior vertebra relative to the inferior vertebra
-termed as slipped
What is the risk factors for spondylolisthesis?
Genetics, trauma, degeneration, spondylolisis PARS fracture, connective tissue disorders, infection, neoplasm
What is the management for spondylolisthesis?
-if it’s high grade and severe nerve compression can result in surgery
-conservative management
What is the aetiology for scoliosis?
-S shaped curve in the spine
-may or may not cause pain
-idiopathic
-congenital(present at birth)
-neuromuscular
What is the management for scoliosis?
Surgery( spinal fusion)
What is whiplash associated disorder?
-after serious pathology is ruled out( spinal cord injury, cervical instability)
-generic term for neck pain followed by sudden force of acceleration/deceleration
What is whiplash associated disorder?
-hearing loss, headaches, double vision, nausea, numbness, eye pain
What is the management for WAD?
Education/reassurance, physiotherapy(ROM, exercise, pain), manual therapy
What is treatment for non-specific spinal pain?
Majority of the spinal pain can be treated conservatively(advice or physio) and majority don’t need X-rays or mri as per nice guidelines
