The Severity Assessment Code (SAC) is the method used by any person who has identified an incident, to determine the appropriate action to take on that incident. The score is ascertained by rating the consequence of the incident and its likelihood of occurrence.
What is RCA and what does it mean
Root Cause Analysis (RCA) is a method or methodology that is used to investigate an incident in order to assist in the identification of health system failures that may not be immediately apparent at initial review

Comes from the business sector and the concept was developed to provide summaries of an organisations achievement of objectives
