Umbilical Cord
1 vein that carries food and oxygen from the placenta to the baby.
2 arteries that carry waste from the baby to the placenta.
Clamp 6 inches and 8 inches from the baby after birth.
Wait 30s after the heartbeat stops in the umbilical cord to cut.
Breach
Two positions to put the mother in for a breach of a limb:
1) Face down, bottom up
2) Knees to the chest and panting.
Braxton Hicks
False contractions.
First pregnancies are usually who this happens to.
Cord Presentation
Same as breach.
Insert two fingers to relieve pressure.
Cover with dry dressing.
Cord Around the Neck
Insert fingers to relieve pressure - continue delivery.
Clamp and cut if delivery does not progress.
Cord on Baby’s Head
Slip over the head (if delivery does not progress).
Do nothing (if delivery progresses).
Controlling Bleeding After Birth
Apply dressing on top of dressing - do not pack the vagina.
Elevate the hips.
Massage the fundus. (upper portion of the uterus). Stimulates the hormone Pitocin. (Vasodilator)
Encourage mother to breastfeed.
Stages of Labor
1) Dilation of cervix.
2) Delivery of the infant.
3) Delivery of the placenta.
Function of the Placenta
Provides oxygen and nutrients.
Removes waste.
Placenta Previa
Placenta blocks the cervical opening.
Causes spotting.
Painful sexual intercourse.
Abrupto Placenta
Uncontrolled contractions.
Spasms in upper back.
Lots of bleeding.
Treat for shock.
Newborn with Heartbeat of 100 or less
Stimulate the baby (wipe down with a towel).
Assist ventilations.
During Birth
Apply gentle pressure to the baby’s head.
Suction mouth to nose with a bulb syringe.
Anterior shoulder delivers first.
Appropriate Delivery Positions
Elevate the buttocks.
Lie with knees drawn up and spread out.
Croup
Disease that causes swelling in the airways and problems breathing.
Causes: Influenza, Parainfluenza virus, RSV, Adenovirus, Enterovirus.
Epiglottitis
Epiglottis swells and blocks airflow into the lungs.
Causes: Burns, Injury, Haemophiles influenzae.
Grand Mal Seizure
Ask how long they’ve been cyanotic.
Febrile Seizure
Get them out of their clothes.
Spontaneous Abortions
Aborts before 20 weeks.
Supine Hypotension
Low blood pressure from lying on your back.
Measure of Contraction
Beginning of one to the beginning of the next.
Women’s circulatory systems expand in the first 20 weeks of pregnancy.
Late in the pregnancy they have hypertension.
Gestational Diabetes: cells are insulin resistant. happens late in the pregnancy.
Meningitis
Inflammation of the fluid and membranes surrounding your brain and spinal cord.
Causes bulging fontanels.
Will have a fever.
Will not be eating or drinking.
Kids will have flue symptoms or pain in the neck.
