What is the ICD10/DSM5 definition of acute stress disorder (And aetiology)
ICD10- transient disorder that develops (in someone with normal MH) in response to exeptional phsysical and mental stress -that usually subsides in a few hours or days
DSM 5- sx last <3days -if over 1 mont -PTSD
onset within minutes
typical case -some has just been told their family died in accident and they do in ASD
Aetiology (use biopsychosocial approach) = physical/sexual assault, death, war, RTA
Difference bewteen PTSD and ASD
Although ASD and PTSD both begin with an acute stressor, ASD differs from PTSD in…
o PTSD is diagnosed based on symptoms in clusters, not in totality
o PTSD has a dissociative sub-type however, in ASD, depersonalisation and derealisation are symptoms
o PTSD includes non-fear-based symptoms (i.e. risky behaviour) whereas ASD does not
Signs and sx of acute stress disorder
Ket features
initial state of Daze (can even be stuper)
constinction of conciousness field
narrowing of attention
inhability to comprehemd stimuli, disorientation
clear hx between stress and reaction
other sx-
automonic sx - tachy, sweating, hyperactive, tachypnea, HTN
partial/total amesia
depersonalisation (arent yousefl)
derealisation (world isnt real)
Ix and Mx of acute stress disorder
Ix -should subside fast - so just full hx
clear hx link between stressor and reactioj
Mx - support and reassurance
BDZ for short term distress (doesnt help PTSD)
but can progress to PTSD
if formal immediate psychoc debrief happens -INCREASE RISK OF PTSD
