A
  • Progressive foot deformity as static subluxation of the 1st MPJ
  • phalanx abducts and ext rotates while met adducts and internally rotates
  • unclear cause, genetic predisposition
  • Four stages

Signs and symptoms

A
  • Pain when in tight footwear
  • Exostosis around 1st MPJ
  • initially will be no pain

Diagnosis

A
  • Hx of hallux pain

- X-ray useful to assess severity of deformity and amount of 1st MPJ degeneration

Treatment

A
  • Well fitting stable footwear and orthoses slow down HAV progression
  • 1st MPJ mobilisation
  • Reduce inflammation and pain, NSAIDS, Icing
  • foot intrinsic exercises
  • medial bunion pads to reduce inflammation
