Radicular cyst

A
  • Most common jaw cyst
  • Develop from periapical granuloma from remnants of pulp→chronic inflammation stimulates cell rests of malassez→Epithelial proliferation→phase of development and growth
  • Fluid filled(expand in jaw by osmotic pressure
  • Cytokines cause local resorption of bone

Clinical presentation of radicular cyst

A
  • Firm swelling at apex of tooth
  • Bone resorption leaving soft swelling that gives way on palpation(shell crackling)
  • Pain on palpation

Investigations of Radicular cysts

A
  • Vitality testing
  • Periapical and panoramic→smooth rounded, unilocular radiolucency w/ radiopaque outline(at apex of non vital tooth)
  • Adjacent structures may be displaced by cyst

  • Cyst in maxilla may occupy crown

  • Definitive surgery
  • Enucleation
  • Treatment of cavity - carnoys solution(kills residual cells in bony spaces)
  • Removal of overlying mucosa(removal of overlying dental lamina rests)

  • Interosseous or in gingiva
  • Benign but can become aggressive and recur
