  • Increases myocardial contractility and stroke volume

  • Cardiogenic shock
  • Septic shock
  • Spinal shock
  • Hypotension with low cardiac output states

  • VF
  • Tachyarrhythmias
  • Hypovolemic shock
  • Phechromocytoma (tumor of adrenal glands)

  • Cardiac arrhythmias

  • Increased myocardial O2 demand
  • Extravasation may cause tissue necrosis

Drug interactions

A
  • Incompatible in alkaline solutions
  • MAOIs enhance effects
  • Bretylium may potentiate effect (antiarrythmic)
  • Beta-blockers may antagonize effects
  • When administered with phenytoin (sz) may cause:
    • hypotension
    • bradycardia
    • sz

Dosage

A
  • Adult & Ped: 2-20 µg/kg/min, titrate for effect
  • Dopaminergic response: (dilates vessels in kidneys; increased urine output): 2-4µg/kg/min
  • Beta-adrenergic response: (positive chronotropic & inotropic): 4-10µg/kg/min
  • **Alpha stimulant/vasoconstict: **10-20µg/kg/min
  • Reverse renal effects/override alpha effects: > 20 µg/kg/min

  • Onset: 1-4 min
  • Peak: 5-10 min
  • Duration: effects cease almost immediately after infusion is shut off
