Exercise Induced Pulmonary Hemorrhage (EIPH) Flashcards

1
Q

Whta is EIPH?

A

strenous exercise associated with extravasation of RBCs from pulmonary vasculature into interstitial space and alveol and airways in DORSOCAUDAL aspect of lung

Q

How is EIPH diagnosed?

A

history and physical exam
endosccopy within 90 minutes of race of workout to detect frank blood in trachea
repeat if not notesd first time endoscopy done
if not then do BAL–blood–phagocytosis by macrophages–hemosiderophage

Q

What are potential causes of EIPH?

A
  1. increased capillary pressure and decreased intrapleural pressure
  2. mechanical constraints of abdominal viscera placed on dorsocaudal lung field
  3. pulmonary hypertension and stress failure of pulmonary capillaries

Q

What does furosemide do that helps with EIPH?

A

reduces severity of bleeding
reduces pulonary capillary pressure
increased urine production
decreases blood and plasma volume and body weight
causes hypochloremia and metabolic aklaosis
decreases cardiac output and vascular pressures

A

furosemide

17
Q

What does furosemide do that helps with EIPH?

A

reduces severity of bleeding
reduces pulonary capillary pressure
increased urine production
decreases blood and plasma volume and body weight
causes hypochloremia and metabolic aklaosis
decreases cardiac output and vascular pressures

18
Q

What are therapyies besides furosemide that help EIPH?

A

nasal strips
post race intinflammatories help IAD
corticosteroids, bronchodilators–do not lessen EIPH
etc others
