Surgical/pathological MRI -Depth of myometrial invasion -Cervical involvement -Lymph node involvement
What are the stages of endometrial cancer?
1A = inner half 1B = outer half 2 = invades cervix 3A = Serosa 3B = Vagina 3C = Pelvic or para-aortic nodes 4 = bladder/bowel/intra-abdominal/inguinal nodes
2 Categories of endometrial cancer
1 = adenocarcinoma
Common
Unopposed oestrogen
Hyperplasia with atypia precursor
2 = Uterine serous and clear cell carcinoma
High grade, more aggressive and poorer prognosis
Generally older women
Serous intraepithelial carcinoma precursor
What is endometrial cancer treatment ?
Early stage - TAH/BSO/ washing
High risk histology = chemo
Advanced = radiotherapy
Palliation = progesterone
What are endometrial cancer cure rates?
1B = 85% 2 = 75% 3B = 30% 4 = 21%
Who gets endometrial cancer?
Post menopausal women High circulating oestrogen -Obesity -Unopposed E2 therapy -PCOS -Early menarche/late menopause HNPCC/Lynch type 2 familial cancer syndrome
What are endometrial cancer symptoms?
Abnormal vaginal bleeding
