16
Q
  1. What are the side effects of this combo (topical + oral)?
A

Topical: irritation, photosensitivity, bleaching; Oral: resistance risk, systemic side effects

17
Q
  1. Are oral tetracyclines safe in pregnancy?
A

No, they may stain teeth and affect fetal skeletal development

18
Q
  1. What acne treatment uses azelaic acid + oral tetracyclines?
A

Moderate to severe acne

19
Q
  1. What are the benefits of azelaic acid + oral antibiotics?
A

Useful for difficult areas; systemic antibiotic coverage

20
Q
  1. What are the risks of using oral tetracyclines?
A

Photosensitivity, resistance, fetal bone/teeth risks

21
Q
  1. Is azelaic acid safer in pregnancy than retinoids?
22
Q

What is the mechanism of benzoyl peroxide?

A

Antiseptic, anti-inflammatory, reduces keratinisation; penetrates duct and releases oxygen radicals

23
Q

What is the mechanism of retinoids (adapalene, tretinoin)?

A

Anti-inflammatory; normalize keratinocyte growth; prevent microcomedones

24
Q

What is the mechanism of azelaic acid?

A

Anti-inflammatory; normalizes follicular keratinisation; reduces comedogenesis

25
What is the mechanism of tetracyclines/clindamycin?
Inhibit bacterial protein synthesis
26
Overall goal MOA of acne treatment
anti inflammatory, reduce keratinization of skin
27
Concern w/ oral tetracyclines
tetracyclines such as doxycycline and lymecycline can cause photosensitivity and should not be used in pregnancy as can cause discoloration of the child’s teeth and affect skeletal development.
28
Acne formation mechanism
4 main events: Excessive sebum production secondary to stimulation by androgens Abnormal follicular keratinization and desquamation resulting in formation of a follicular plug. Proliferation of the anaerobe Gram positive bacteria Cutibacterium acnes Inflammation.
29
Concern for treatment with Isotretinoin
Isotretinoin- high risk of serious congenital malformations, and any use in females must be within the conditions of the Pregnancy Prevention Programme (usually IUD/implant) and initiated by a dermatologist.
