A

Loss not from the kidneys but elsewhere: diarrhoea, vomiting, fistulae, burns, small-bowel obstruction, trauma, CF, heat exposure

If urine osmolality >500mmol/kg: inappropriate ADH (e.g. lung Ca)
If not: water overload, severe hypothyroidism, glucocorticoid insufficiency
