absorption varies depending on formulation. Poorly soluble in aqueous. The original drug
Fosphenytoin
dissolves easier than phenytoin. converted into phenytoin
Phenytoin pharmacokinetics
Dose dependent (zero order). Can be saturated.
Phenytoin mech of action
- blocks and prolongs the inactivated state of voltage gate Na channels
- enhances release of GABA
- Prevents seizure propagation
Phenytoin side effects
Gingival hyperplasia is the big one

Carbamazepine mech of action
similar to phenyotoin-inhibition of voltage gated Na channels
Carbamazepine metabolism
Potent inducer of P450s and its own metabolism
Oxacarbazepine
Newer, similar to carbamazepine. Shortr half life, active metabolite has longer duration and FEWER drug interactions
Carbamazepine clinical use
- General clonic tonic seizures
- Partial seizures
- Trigeminal neuralgia-drug of choice
- Manic phase of bipolar disorder
Carbamazepine side effect
SIADH-Syndrome of Inappropriate ADH secretion.
Teratogenic-cleft palate and spina bifida
Phenobarbital mech of action
Enhances phasic GABA receptor responses-increases opening time of Cl channel
Phenobarbital clinical use
- Neonatal seizures

Primidone
Metabolized by liver to phenobarbital
Ethosuximide
Pure petit mal drug
Ethosuximide mech of action
blocks presynaptic T type Ca channels—>blocks high frequency firing of neurons
Valproic acid
Inhibition of presynaptic T cell Ca channels —> blocks neuronal firing
inhibition of GABA transaminase
Valproic acid clinical use
Absense of seizures (petit mal) drug of choice
Valproic acid adverse effects
Hepatotoxic syndrome
Not meant for kids under 5
Teratogenic risk
Diazepam
Initial agent for status epilepticus
Diazepam mech of action
Potentiates GABAa responses by increasing frequency of channel opening
Diazepam clinical use
Status epilepticus. Shorter acting, give with another drug like phenytoin
Lorazepam
Longer duration of action than diazepam, so can be used alone
Gabapentin
analog of GABA
