what does this CPG treat?
cardiogenic APO secondary to CCF or LVF- non-cardiogenic or non symptomatic APO should not be treated under this guideline
what should be done for non-cardiogenic APO?
includes smoke inhlation, drowning, and anaphylaxis
result of altered permiability and should b treated with supplimnetal oxygen and assited ventialtions - not requiring nitrates
what are the contraindications to CPAP?
airway - inability to mange own airway (altered concious, active vomitting, excessive secretions
breathing - hypoventilation, untreated tension pnumothorax
circulation - heamodynaically unstable - sevre hypotension, vascular arythmias
others - injuries preventing mask application
what are the indications to remove CPAP??
ineffective - cardiac resp arrest, intollerence, decreasing resp effort, nill improvment after an hour of treatemnt
deteriorating vitals
risk to paintent - loss of airway, copous secreations, active vommiting, paramedic judgment
what is the treatment of symptometic cardiogenic pulmonary odeama?
sob and crackles
GTN 600mcg if SBP above 100
or 300 mcg if no previous admin, boaderline bp or small
repeate 300 or 6000 mcg at 5 mins titrated to pain/side effects
GTN path 50mg 0.4mg/hour upper torsoe - remove patch if BP falls below 100
if there is no improvment after GTN or if there is full feild APO what is the treateent?
CPAP 10cm h20 - suction and assisted ventiations if required
