how to assess the extent of a burn
wallace rule of nines
- head + neck = 9%
- each arm= 9%
- each anterior leg= 9%
- each posterior leg= 9%
- anterior chest= 9%
- posterior chest= 9%
- anterior abdomen= 9%
- posterior abdomen= 9%
presentation of superficial epidermal burn
red and painful
dry
no blisters
presentation of partial thickness burn
pale pink, painful, blistered
slow capillary refill
presentation of partial thickness (deep dermal) burn
typically white but may have patches of non-blanching erythema
reduced sensation
painful to deep pressure
presentation of full thickness burn
white/ brown / black in colour no blisters, no pain
initial management of severe burns
airway assessed:
- smoke inhalation causes oedema- early intubation
IV fluids for children with burn > 10% and adults > 15%
urinary catheter
analgesia
haematuria after extensive burns
curling’s ulcers
acute gastric ulcers develop in response to severe physiological stress
