3 conditions for being Absent without authorization.
Absent prior to receiving authorization
Absent even though leave was denied
Absent without contacting their supervisor
What happens after 3 days of Abesent without Auth
Indefinite suspension
When do you have to submit a MODS form?
Return after being off for 2 shifts or 5 days
After being off for 10 shifts or 20 days and 10/20 thereafter
Anytime there is a change to member’s condition
Who do you give the MODS to when you return?
You give it to your BC or Supervisor, they turn it over to Wellness
After you are cleared by the doc with a MODS form, what do you do?
Report to Wellness before, during or after your first shift back with the completed MODS form
How many days can you be off for Immediate Family illness?
2 shifts or 4 days. After that,notify BC or Supervisor, and then look into FMLA, Cat leave, etc
Emergency leave is used for?
DIF
How much time for Emergency/DIF leave?
53 hr 4 days or 48 hours
40 hr 5 days or 40 hours
For Military, USAR, Jury duty, who do you send the paperwork to?
BC or Supervisor, company officer or supervisor, and payroll
How many days do Military/USAR members get?
15 days ( be careful, this is only one component)
How many hours off do 53 hr/wk members get if they are in the military or USAR? 40 hr/ wk members?
228 (15x 12) plus 48
120 for 40 hr/wk members
October 1- September 30
Leave requests must be on what and have what?
Official letterhead and include, date, time, and location of activity, and the name, signature, title, and telephone number of authorizing official
When do you have to turn in your jury summons?
Shift before

The fire chief
