4. Coming to full duties after being on modified duties due to off the job injury
5. Coming back to full duties after chest pans
6. Submitted monthly if off
7. Submitted monthly if on modified duties

the corporation currently offers up to $1500 a year in reimbursement for educational request. this is a corporation program and requires corporate approval.
