Q

If bacterial meningitis is suspected…Tx?

A

Start 3rd gen cephalosporin (Ceftriaxone - broad spec) immediately with Vancomycin (especially for Nsurg pts, for S. Aureus.

Add ampicillin if over 55 or immunocompromised (for possible Listeria)

Steroids (dexamethasone) to reduce vasogenic swelling.

Q

Altered consciousness, seizures, focal Neuro symptoms and normal CSF might indicate:

A

Viral Encephalitis

17
Q

The most common cause of viral encephalitis is ______ ____ __.
The classic sign is ___________________________.

A

Herpes Simplex I due to reactivation of virions in Trigeminal nerve.

The classic finding is MEDIAL TEMPORAL LOBE inflammation on MRI.

Q

A cause of encephalitis that has a variable time course due to retrograde transport and results in hydrophobia is:

20
Q

Two common causes of bacterial encephalitis are:

A

Neurosyphillis

Lyme disease

21
Q

The VDRL test would be used to check the CSF if ____ was suspected in a pt with meningitis, cranial nueritis, sensory ataxia, and dementia.

A

The VDRL test checks the patients serum for the presence of anti-cardiolipin antibodies, which are porduced in response to syphillis infection. This is the test for treponema pallidum, syphillus.

22
Q

T/F:
A brain abscess will cause fever
A brain abscess will cause increased ICP

A

F
F
brain abscess will not cause fever or ICP

23
Q

An AIDS patients CT shows multiple ring-enhancing lesions. His CD4 count is low.
Dx?
Tx?

A

Cerebral Toxoplasmosis.

Treat with Pyrimethamin + Sulfadiazim

24
Cysticercosis is: | On CT:
An invasion of brain by pork tapeworm taenia solum. Endemic in Latin America. On CT shows enhancing calcified lesions.
25
Progressive Multifocal Luekoencephalopathy is caused by: Shows __?__ on imaging Tx?
Is caused by reactivated JC virus in immunocompromised (HIV) patients Shows White matter lesions on imaging. Can only be treated by restoring the immune system.
26
The most common cause of fungal meningitis is: It can be diagnosed with: CSF would show:
Cryptococcus Neoformans Dx with India ink stain of CSF CSF will also show eosinophilia.
27
Vacuolar Myelopathy is:
Caused by direct HIV invasion to spinal cord resulting in ataxia
28
Botulinum toxin blocks _____ | Tetanus toxin blocks ______
Botulism - blocked Ach release | Tetanus - blocks GABA + glycine release by inhibitory interneurons
