Remove, eliminate desensitization from environmental triggers, irritants, or causative factors - dust, pollutant, air, pollen, dirt, mite, pollutant, allergen, animal fur, strong odors, smoke, perfume, food allergy, drug allergy, physical activity, quiet environment, change job (if exposed to occupational allergens).
INEFFECTIVE AIRWAY - supplemental oxygen therapy, metered-dose inhaler, nebulization therapy, deep breathing exercises, pursed-lip breathing, chest physiotherapy, endotracheal intubation and mechanical ventilation (for respiratory arrest), relaxation techniques
SUPPORTIVE NURSING CARE - adequate rest periods, increase oral fluid intake (2-3L/day), drink warm fluid, good hydration, good nutrition
BRACHIAL ASTHMA
Close Monitoring - vital signs, ABG analysis and pulse oximetry, lung sounds, heart sounds, ECG
IMPAIRED GAS EXCHANGE (hypoxemia) - Low flow supplemental oxygen therapy via nasal cannula, endotracheal intubation, mechanical ventilator, elevate head of the bed, Fowler’s position, frequent turning, coughing exercises, deep breathing exercises, incentive spirometry
IMPAIRED CIRCULATION - avoid flexion of legs, prolonged activities (prolong sitting, prolonged standing, severe hip flexure, constrictive clothing, tight garments, massage legs, weight reduction), DVT prevention, compression anti-embolic stockings, anticoagulants therapy, early ambulation, hemodynamic stability, cardiopulmonary resuscitation, intravenous fluids.
Prepare patient for possible surgery - pulmonary embolectomy (surgical intervention to remove embolus and obstruction in the pulmonary arteries to patient with hemodynamic instability).
PULMONARY EMBOLISM
- Provide relief from pruritus. Administer Loratidine 10mg per Orem once a day.
- Asses for secondary bacterial infections and treat as needed.
- Provide well balanced diet especially adequate amounts of Vit.C , protein and collagen to support skin recovery.
