first line for agitation in palliative
haloperidol but if in terminal phase of illness then midazolam

when increasing dose in palliative should increase by –

divide by 10
oral morphine to oral oxycodone
divided by 1.5-2
12mg fentanyl patch equates to
20mg oral morphine
buprenorphine 10mg patch equates to 24mg morphine
oral morp to subcut morph
divide by 2
avoiding secretions
avoid fluid overload - stop fluids
advise family they are not troubled by secretions
secretions/ bowel colic
hyoscine butyl bromide
preferred opiod in a syringe driver
diamorphine
