What are the care objectives for Seizure?

Why do we use Midazolam in Seizures?
Fast acting benzo that helps stop seizures quickly - by enhancing the effect of GABA, the brains main inhibitory neurotransmitter - calming the excessive electrical activity in the brain
Does the O2 therapy guideline apply to a seizure presentation?
Yes - in status epilepticus
As it is a “critical illness”
Are there any relevant CWI’s in seizures
- Airway considerations
- Suction
- IM
- IV
- Preparing medications for adminsitration
What are 3 differential diagnoses for seizure?
- Alcohol withdrawal seizure
- Head injury (chicken or egg)
- Eclampsia (in pregnant women)
What are some care pathway/treatment options to consider
- Reassurance
- If patient has fully recovered consider treat and refer pathway
- Transport to hospital
- Reassessment
Now in the back of the ambulance - are there any other considerations?
MICA
* Can administer IV midazolam
* ETT
Consult/Clinician
* For seizures other tahn GCSE, midaz may only be administered following consultation via the AV clinician
* Can consult for further doses
* Continue to look for cause and reverse if able
* What is your scene time?
* How far is MICA - am I waiting or loading and going?
* How far is the hospital?
