What is end of life care
Support for people who are in the last months or years of their life
What is palliative care
If you have an illness that cannot be cured, palliative care makes you as comfortable as possible, by managing your pain and other distressing symptoms
What do the following terms mean
Hospice
Expected death
Sudden death
Grief
Bereavement
Hospice-a place for treatment/ to pass away
Expected death- death that they knew was coming and were more prepared (terminal illness)
Sudden death- an accident happened, unexpected and the family couldn’t prepare themselves
Grief- mental distress over a loss
Bereavement- part of grief, after death happened, the feelings you experience
what are the 5 stages of grief according to the kubler-Ross model
Denial, anger, bargaining, depression, acceptance
What might the individual be feeling during the denial stage of grief
Disbelief, shock, feeling numb, can’t believe this has happened, don’t want to believe its true, avoidance, confusion, fear, isolation
What might an individual be feeling during the anger stage of grief
Enraged, asking why this has happened, frustration, irritation, anxiety
What might an individual be feeling during the bargaining stage of grief
Struggling to find meaning in the situation, questioning ‘what if’ and ‘if only’, telling ones story, reaching out, struggle to find meaning
What might an individual be feeling during the depression stage of grief
Overwhelmed, helplessness, hostility, flight, realisation of the bleak reality of the situation, a sense of of hopelessness
What might an individual feel during the acceptance stage of grief
A feeling of coming to terms with what has happened and accepting your new reality and beginning to re-engage with others, exploring options, new plan in place, moving on
What are the signs that someone is in the active stage of dying
-sleeping more
-laboured breathing (death rattle)
-skin turns grey/ discoloured
-visual change in their appearance
-vitals change
What is advanced care planning
Understanding the patients wishes, value and beliefs. The goal is to help ensure people receive medical care that is consistent with their goals and preferences during serious/chronic illness
How would you support people with bereavement
-provide safe and comfortable environment, suitable resources (tissues, refreshments)
-provide emotional support (allowing them to cry and talk)
-use active listening techniques
What is your role in end of life care
-manage pain with meds, hot water bottle, warm flannels, positioned right
-music, keep them company, communication
-mouth care, washing, dressing, moisturise,
-patient cords updated with patients wishes
