Tolerância imunológica (intro)

A
  • Refere-se à ausência de resposta a um Ag que é induzida por exposição prévia a esse mesmo Ag. Tolerância imunológica pode então definir-se como a ausência de resposta imunológica agressiva a um conjunto de Ags, num sistema imunológico competente.
  • Falência de mecanismos de manutencão de tolerância ao próprio (self tolerance) resulta em respostas imunológicas adquiridas contra antigénios do próprio à autoreatividade; e as doenças causadas designam-se por doenças autoimunes.
  • Self/non-self descrimination: capacidade do SI de reconhecer e responder a Ag “non-self” (exógenos) e não a “self” (do próprio).
  • Tolerância/autoimunidade são propriedades do organismo e não de células ou moléculas.
  • A elucidação dos mecanismos de tolerância ao próprio são fundamentais para compreender a patogénese de doenças autoimunes e desenvolver novos tratamentos das mesmas. Estes mecanismos estão também envolvidos na tolerância a microrganismos comensais, a Ag fetais e alergéneos.

Características da tolerância imunológica

A

• Os mecanismos de tolerância eliminam ou inativam linfócitos B e T que expressam recetores de Ag com elevada afinidade para Ags do próprio.

–> Processo de recombinação V(D)J é aleatório; alguns linfócitos podem reconhecer Ag do próprio; mecanismos de tolerância imunológica previnem a resposta imunológica mediada por estes linfócitos.

  • Existem mecanismos de tolerância imunológica centrais, que atuam em linfócitos T e B imaturos durante o seu processo de diferenciação no timo e na medula óssea, assim como mecanismos de tolerância periféricos, que atuam em linfócitos T e B maturos na periferia.
  • Tolerância central ocorre durante o estadio de diferenciação de linfócitos no qual uma interação de elevada afinidade com Ag do próprio pode levar à apoptose ou substituição do receptor de Ag por outro não reativo (apenas para LB).
  • Mecanismos periféricos de inducão de tolerância incluem indução de anergia ou deleção clonal, pelos quais linfócitos maturos que reconhecem Ag do próprio se tornam “não responsivos” após nova exposição ao Ag ou morrem por apoptose, respetivamente.
  • Ignorância clonal é também um mecanismo de manutenção de tolerância na periferia. Ignorância ocorre quando os Ag estão sequestrados em locais designados por “imunologicamente privilegiados” como o olho, cérebro, útero e testículos. Alternativamente, a concentração destes Ag pode ser demasiado baixa para ativar eficientemente um linfócito.
  • Anergia, deleção e ignorância são mecanismos passivos de tolerância periférica.
  • A tolerância periférica é também mantida por células T reguladoras (Treg) que suprimem a ativação de linfócitos específicos para autoantigénios (aka Ag do próprio). A manutencão de tolerância mediada por Tregs é considerada um mecanismo ativo de tolerância periférica.
  • Uma vez que os linfócitos CD4+ auxiliares controlam a resposta imunológica a virtualmente todos os Ag proteícos, indução de tolerância nestas células é usualmente suficiente para prevenir respostas imunológicas celulares e humorais a proteínas do próprio.
  • A indução de tolerância imunológica é uma possível estratégia terapêutica para prevenir respostas imunológicas indesejáveis, sendo explorada na prevenção e tratamento de autoimunidade, na aceitação de transplantes de orgãos, tratamento de alergias, etc.

Tolerância central e periférica a Ag self

A

Central tolerance: Immature lymphocytes specific for self antigens may encounter these antigens in the generative (central) lymphoid organs and are deleted; B lymphocytes change their specificity (receptor editing); and some T lymphocytes develop into regulatory T cells. Some self-reactive lymphocytes may complete their maturation and enter peripheral tissues.

Peripheral tolerance: Mature self-reactive lymphocytes may be inactivated (anergy) or deleted (apoptosis) by encounter with self antigens in peripheral tissues or suppressed by regulatory T cells (ignorância clonal).

AIRE (autoimmune regulator) is a transcription factor that mediates expression of many self antigens in the medullary epithelial cells of the thymus (mTECs), required for deletion of selfreactive thymocytes.

In the absence of functional AIRE, these self-reactive T cells are not eliminated, they can enter tissues where the antigens continue to be produced and cause injury.

  • These antigens are administered in Complete Freunds adjuvant (CFA), usually with a particular strain of dead mycobacteria H37RA: this gives potent dendritic cell activation. Mouse or rat strain used is important.
  • By injecting healthy mice with some self-antigen, such as myelin basic protein in adjuvant, quiescent autoreactive T cells that have escaped deletion are primed and cause autoimmune attack on tissues expressing these proteins

If a T cell recognizes antigen without strong costimulation, the T cell receptors may lose their ability to deliver activating signals (signaling block), or the T cell may engage inhibitory receptors, such as cytotoxic T lymphocyte–associated protein 4 (CTLA-4), that block activation
=> unresponsive (anergic) T cell

TCR ligation in the absence of costimulation (ex Anti-CD3 with no co-stimulation) leads to inability to express effector functions like cytokine secretion, and makes the cell unresponsive to further stimulation.

Deleção

A

Mechanisms of apoptosis of T lymphocytes. T cells respond to antigen presented by normal antigen-presenting cells (APCs) by secreting interleukin-2 (IL-2), expressing anti-apoptotic (pro-survival) proteins, and undergoing proliferation and differentiation. The anti-apoptotic proteins prevent the release of mediators of apoptosis from mitochondria.

Self antigen recognition by T cells without costimulation may lead to relative deficiency of intracellular anti-apoptotic proteins, and the excess of pro-apoptotic proteins causes cell death by inducing release of mediators of apoptosis from mitochondria (death by the mitochondrial [intrinsic] pathway of apoptosis).

Alternatively, self antigen recognition may lead to expression of death receptors and their ligands, such as Fas and Fas ligand (FasL), on lymphocytes, and engagement of the death receptor leads to apoptosis of the cells by the death receptor (extrinsic) pathway.

A

Regulatory T cells (Tregs) are generated by self antigen recognition in the thymus (sometimes called natural regulatory cells) and (probably to a lesser extent) by antigen recognition in peripheral lymphoid organs (called inducible or adaptive regulatory cells).

The development and survival of there Tregs require IL-2 and the transcription factor FoxP3.

In peripheral tissues, Tregs suppress the activation and effector functions of other self-reactive and potentially pathogenic lyphocytes. (inhibition of T cell responses and of other cells).

Ag MONOVALENTES: Weak recognition of self antigens in the bone marrow may lead to anergy (functional inactivation) of the B cells.

Environmental triggers, such as infections and other inflammatory stimuli, promote the influx of lymphocytes into tissues and the activation of self reactive T cells, resulting in tissue injury.

DAI específicas de orgãos + exemplos

A

-> Específicas de orgão: Função efetora visa autoantigénio(s) restrito(s) a orgão(s) particular(es). Nalguns casos a destruição mediada pelo sistema imunológico do orgão alvo leva à cessação da reação autoimune.

Exemplos clássicos:

. Tiroidite de Hashimoto- Linfócitos T e autoanticorpos reconhecem proteínas específicas da tiroide, levando à destruição do orgão e hipotiroidismo.

. Diabetes tipo I- Início em idade muito jovem, causada por LT que reconhecem componentes das células beta pancreáticas, causando destruição destas com consequente perda de produção de insulina e dependência de insulina exógena.

. Esclerose múltipla- DAI neurológica causada por células T autoreativas que reconhecem componentes do SNC, levando à destruição da bainha de mielina que envolve os nervos e disfunção neurológica progressiva.

. Miastenia gravis- Causada por autoanticorpos que reconhecem recetores de acetilcolina nos músculos, bloqueando a ligação deste neurotransmissor e levando à
fraqueza progressiva dos músculos.

DAI sistémicas + exemplos

A

Sistémicas: Causa inflamação em múltiplos orgãos porque os autoAgs alvo estão presentes na maior parte das células do corpo. Tendem a ser crónicas se não tratadas (autoAgs não são eliminados).

Exemplos clássicos:

. Lupus sistémico eritematoso- resulta da produção de anticorpos dirigidos contra componentes celulares comuns, especialmente proteínas nucleares, levando à formação de complexos imunes que bloqueiam vasos sanguíneos e causam progressivamente falência de orgãos.

. Artrite reumatóide- DAI que aparece em idade mais tardia, causada por anticorpos contra antigénios proteícos citrunilados (citrunilação é a conversão do aminoácido arginina em citrulina), induzindo a formação de complexos imunes que se depositam nas articulações e levam à destruição progressiva destas via atividade do sistema do complemento.

For four important autoimmune diseases, the figure lists the roles of T cells, B cells, and antibody. In some diseases, such as SLE, T cells can have multiple roles such as helping B cells to make autoantibody and directly promoting tissue damage. B cells can have two roles as well presenting autoantigens to stimulate T cells and secreting pathogenic autoantibodies.

Mecanismos pelos quais os microrganismos podem causar autoimunidade

A

Normally, an encounter of mature T cells with self antigens presented by resting tissue antigen-presenting cells (APCs) results in peripheral tolerance.

Microbes may activate the APCs to express costimulators, and when these APCs present self antigens, the specific T cells are activated, rather than being rendered tolerant.

Some microbial antigens may crossreact with self antigens (mimicry). Therefore, immune responses initiated by the microbes may become directed at self cells and tissues.

• Immunologic tolerance is unresponsiveness to an antigen induced by the exposure of specific lymphocytes to that antigen. Tolerance to self antigens is a fundamental property of the normal immune system, and the failure of self-tolerance leads to autoimmune diseases. Antigens may be administered in ways that induce tolerance rather than immunity, and this may be exploited for the prevention and treatment of transplant rejection and autoimmune and allergic diseases.

• Central tolerance is induced in the generative lymphoid organs (thymus and bone marrow) when immature lymphocytes encounter self antigens present in these
organs. Peripheral tolerance occurs when mature lymphocytes recognize self antigens in peripheral tissues under particular conditions.

• In T lymphocytes, central tolerance occurs when immature thymocytes with high affinity receptors for self antigens recognize these antigens in the thymus. Some
immature T cells that encounter self antigens in the thymus die (negative selection), and others develop into FoxP3+ regulatory T lymphocytes (Tregs) that function to control responses to self antigens in peripheral tissues.

• Several mechanisms account for peripheral tolerance in mature T cells. In CD4+ T cells, anergy is induced by antigen recognition without adequate costimulation or
by engagement of inhibitory receptors such as CTLA-4 and PD-1. Tregs inhibit immune responses by multiple mechanisms. T cells that encounter self antigens
without other stimuli or that are repeatedly stimulated may die by apoptosis. Autoreactive T lymphocytes can ignore the presence of its cognate Ag if it is present at too low concentration or expressed in immunologically privileged sites.

• In B lymphocytes, central tolerance is induced when immature B cells recognize multivalent self antigens in the bone marrow. The result is the acquisition of a new
specificity, called receptor editing, or apoptotic death of the immature B cells.

• Mature B cells that recognize self antigens in the periphery in the absence of T cell help may be rendered anergic and ultimately die by apoptosis or become
functionally unresponsive because of the engagement of inhibitory receptors.

  • Autoimmunity results from inadequate self-tolerance or regulation of lymphocytes.
  • Most autoimmune diseases are polygenic, and numerous susceptibility genes contribute to disease development. The greatest contribution is from MHC genes; other genes are believed to influence the selection or regulation of self-reactive lymphocytes.

• Infections may predispose to autoimmunity by several mechanisms, including enhanced expression of costimulators in tissues and cross reactions between
microbial antigens and self antigens. Some infections may protect individuals from autoimmunity, by unknown mechanisms.
