A

What is the pathophysiology of paracetamol poisioning?

A

Paraectamol metabolised in liver mainly by conjugation with glucuronide & sulphate, but when they are saturated, also by CYP450 which produces toxic metabolite. Toxic metabolite detoxified in conjugation with hepatic glutahione. When glutahione depleted, toxic medabolite binds to hepatocyte protiens & damages.

How does paracetamol poisoning treatment work?

A

Acetylcysteine increases glutahione to conjugate with toxic metabolite.
