when should you perform a comprehensive health assessment?
it may be performed when a patient makes initial contact within a health care system
what is a comprehensive health assesment?
it encompasses physical, psychologcal, social, and spiritual dimensions of living.
what are the components of a focused health assesment?
they include performing general serveys, taking vital signs, and assessing specific areas that relate to the problem
what is Subjective Data?
imformation on symptoms, feeling, perseptions, preferences, values, and information, that only the patient can state and validate

directly observed or measured, such as vital signs or appearance

a functional health assessment evaluates the effects of the mind, body, and environment in relation to a persons ability to perform the tasks of daily living

a system for collecting data in an organized manner starting from head and proceeding systematically downward to the toes
what is body systems framework?
it focuses on the pathophysiology involved with in specific body systems
what is secondary data?
includes sources of data other than the patient, such as history or other health care providers
what is primary data?
information gathered directly from the patient during the initial stage of assesment

a goal-directed conversation between nurse and patient
