Where do you do a heel stick?
On the lateral parts of the heel
Why do you do a heel stick on the lateral part of the heel ?
There are less nerves on the lateral part of the heel than the medial part of the heel
What size lancet do you use for a heel stick?
<2mm
How do you increase blood flow to the heel prior to a heel stick ?
Use a warm wash cloth or heat pack.
How can you decrease pain in the heel prior to a heel stick?
Use prilocane/ lidocane cream.
How long should you leave lidocane on the heel before wiping it off?
30-60 minutes
What do you cover the heel with after applying lidocaine ?
Bio-occlusive dressing
What will the heel look like if the lidocaine cream worked?
Red and blanchable
Prior to performing a heel stick what should you do ?
Wipe with alcohol and do not use a tourniquet

Body outlines for pain assessment. Have the child color the areas on the outline to show you where the pain is. > 7 years old
What is the age you use the APPT?
7 years of age. For adolescents

Facial expression, Leg movement, Activity, Cry, Consolability.

What is Atraumatic care ?
Anything we can do to decrease the stress, anxiety, and pain of a child while hospitalized.
Pharmacological atraumatic care
Tylenol, ibuprofen, morphine, dilaudid.
Non-pharmacological atraumatic care
Swaddling, distractions, non-nutritive sucking, skin to skin, ice packs.
What must you do with Atraumatic care?
Must try all options to decrease child’s pain.
Consent
Parent/ guardian must consent for adolescents till age 18.
Who is/ what makes you eligible to consent under the age of 18?
Emancipated minors, pregnancy, marriage, military, court ordered.
Assent
“Right thing to do”, NOT legally binding.
Who is eligible for Assent?
Adolescents over the age of 7. Need to be able to understand what is going on prior to procedure or treatment. Often seen in research.
