Malignant tumor of the kidney that arises from embryonal cells
Most cases of wilms are
Solitary lesions, 6% involve both kidneys

Presenting symptoms
Abdominal pain (37) hyper tension, hematuria, fever
Typically presents as a
Painless abdominal mass.
Grows rapidly
Symptoms often reflect the pressure put on
Abdominal organs.
Wilms tumors have a tendency to
Rupture
Unlike patients with Neuroblastoma
Children with wilms do not appear severely ill
A differential diagnosis includes
Neuroblastoma
Lymphoma
Sarcoma
Liver tumor
What is the first step in diagnosis?
Abdominal ultrasound
CT scan of chest, abdomen, pelvis
To assess the extent of disease and evaluate lymph nodes and lungs
What is the primary site of Mets
Lungs, must be assessed
Biopsy is
Contraindicated because they do tend to rupture
Can invade or Mets into
Liver and inferior vena cava
Classified
Favorable (90)
Or unfavorable/ anaplastic (10%)
Unfavorable histology
Focal anaplasia
Diffuse anaplasia
Unfavorable histologies are associated with
Aggressive behavior and frequent Mets to brain bone.
