All individuals needing an identification card will be required to submit to a background investigation. The following reasons will exclude the individual from receiving an identification card: (4)
1) Conviction of any Forcible Felony
2) Conviction or charge pertaining to any drug or theft, whether felony or misdemeanor
3) If there is any pending felony criminal or theft case against the individual
4) Any exceptions to the above must be approved by an Assistant Chief or higher

Sheriff’s Office personnel and employees, who have had their identification cards revoked, but have a need to enter the PMB, will be issued a ____ border card with the approval of the _____________.

Any employee whose identification card is either lost or stolen shall immediately contact the Security Coordinator via email at SecurityCoordinator@jaxsheriff.org.
Additionally, the employee shall do the following: (4)
1) Complete a General Offense Report
2) Complete Identification Card – Lost or Stolen Form (P-0695) and send through your chain of command to the Assistant Chief
3) Pay $5 fee at the Public Counter and get receipt
4) Bring the Identification Card – Lost or Stolen (P-0695) form and Public Counter receipt to Room 216A to the Security Coordinator or designee
Retirees may be denied a retiree identification card and/or access to any Sheriff’s Officemcontrol facility beyond publicly accessible areas for any reason determined by the administration, including but not limited to: (4)
1) The retiree separated from the Sheriff’s Office while under investigation for criminal misconduct or while under administrative investigation regarding serious misconduct
2) The retiree is determined to be a danger to himself or others
3) The retiree, after separating from the JSO in good standing, becomes the subject of an active criminal investigation or is convicted of a crime
4) Any deviation from the above must be approved by the Undersheriff or his designee
